2. Complete the form
Use the child’s name as shown on the birth certificate and accurate guardian contacts.
3. Attach documents
Two passport photos and a birth certificate are required; previous-school documents apply when relevant.
4. Submit and keep the reference
Online submission generates a reference. Submitting does not mean a place is accepted.
5. Confirm payment instructions
Confirm the form fee and bank accounts with the office before paying. Admin verifies payments.
